(1) The menu there can be controlled in Appearance -> Menus: https://prnt.sc/fw_9eBdKZ6Mu

(2) Yes, commissions will be calculated only for products in category A. If an item is in B, and not in A, then it will not be calculated.
Example:
10% commission rule for category A:
Product 1 - category A - $500. Commission = $50
Product 2 - category B - $500. Commission = 0
Product 3 - category A + category B - $500. Commission = $50
